If you haven't already paid, see the details at the end.  In the case of groups getting full, priority is given to those who have PAID.

We do have a morning tea roster for adults - so please indicate to us if you have important dietary conditions.  Otherwise, BYO morning tea / snack box for your child/ren.

GWAC collects and maintains information about its parishioners and others to whom it ministers. GWAC is committed to protecting the privacy of all individuals who provide personal information to us in accordance with The Privacy Act 1988, which governs the collection, use and disclosure of personal information. GWAC treats all personal information as confidential and takes all reasonable steps to protect the personal information we hold from misuse and loss and from unauthorised access, modification or disclosure.

CONSENT:

· I have read the Confidentiality Statement above.
· I have read and understood the Glen Waverley Anglican Church Playgroup Information Brochure.
· I understand that the Glen Waverley Anglican Church Playgroup will contain some Christian content as outlined in the Information Brochure.
· I agree to participate in and adhere to the Playgroup Procedures and Behaviour Guidelines.
· I understand that my child/ren must be accompanied by a parent or nominated carer who is responsible for my child/ren and their safety whilst they are attending Glen Waverley Anglican Church Playgroups.
· I agree to indemnify and hold harmless the Glen Waverley Anglican Church, its staff, leaders, playgroups and helpers against all claims, demands, suits and liability of whatever arises out of any injury to my child or myself whilst at playgroup, or any associated playgroup activity.
· I acknowledge that my child/rens photo may be taken by other parents/carers who are primarily taking photos of their children.

PERMISSION:

I consent to my childs participation in playgroup. I will encourage my child to participate and co-operate with the leaders and other participants at all times.

I understand there may be photographs and/or video footage taken of my child during ministry activities and am willing for my child to be so filmed in appropriate settings. I am also willing for these photos or footage to be used to promote the ministry in a way that does not identify their name or details and are not published on a website or distributed in an electronic format. My child is also willing for photographs and/or video footage to be taken of them.

Registration fees for Playgroup in 2024: Either an up-front yearly payment of $240 (discounted), otherwise you can pay $80/term at the start of each term (a total of $320 per year).
